This form is intended for students to submit their Certificate of Eligibility and the sponsor’s DD-214 (Member-4 copy). If you change your benefits, please resubmit this form along with the updated Certificate of Eligibility.

Please upload your 2026-2027 Cal Vet Fee Waiver letters here. This award letter will be good for Fall 2026, Spring 2027, and Summer 2027. Basic tuition will be waived for each semester; campus fees will still need to be paid through the students portal. Cal Vet Fee Waivers are only accepted by completion fo this form for SDSU Imperial Valley Campus and cannot be applied to programs offered through our SDSU Global Campus and Main Campus.
